IMPORTANT NOTICE ABOUT FINAL REPORTS:

If you received a grant that was made prior to November 1, 2010, please use the form embedded in the links to the right.

If you received a grant that was made after November 1, 2010, the Foundation will e-mail you a copy of your final reporting form two months prior to its due date. The form will be sent to you when your initial grant payment is made so that you may see the questions that will be asked at the end of the grant period.

Small General Operating Support Grants

Reporting Requirements:

Current grantees with a one-year grant must complete and submit a Final Report 15 months from the date the payment is issued. Current grantees with a two-year grant must complete and submit a Final Report 15 months from the date of the final payment.

Note:  Even if a grant has a matching payment that is made after the initial payment, the Final Report is still due 15 months from the date of the initial payment.

Current grantees who plan to submit a new (or renewal) application prior to the end of their current grant must submit a Progress Report when they submit a new grant application. Unlike with Final Reports, all small grantees, regardless of when their grant was made, should use the Progress Report form embedded in the link to the right.

Note:  Organizations that submit a progress report are still required to turn in a Final Report when it is due.

To request payment of a matching grant, grantees must complete and submit the Matching Grant Form.

Grant Extensions:

To request a grant extension, grantees must complete and submit the Grant Extension Request Form.
