IMPORTANT NOTE ABOUT REPORTS FOR MULTI-YEAR GRANTS:

If you received a grant that was made prior to November 1, 2010, please use the forms embedded in the links to the right.

If you received a grant that was made after November 1, 2010, the Foundation will e-mail you a copy of your progress and final reporting forms two months prior to their due dates. The final reporting form will be sent to you when your initial grant payment is made so that you may see the questions that will be asked at the end of the grant period.

Two Year Strategic Grants

The following applies to strategic grants; if you are a small grant recipient, please click here.

Reporting Requirements:

Grantees with two-year grants must submit a Progress Report 12 months from the date of the first year’s payment.

Two year strategic grant recipients must submit a Final Report up to 15 months after the second year’s payment is issued. 

Note:  Even if a grant has a matching payment that is made after the second year payment, the Final Report is still due 15 months from the date of the second year payment.
