Downloads marked "pdf" are in Adobe Acrobat (.pdf ) format. You may need to download Adobe Acrobat Reader in order to view these files. If you would like to receive a hard copy of a form or publication, you may complete and submit the Document Request Form at the bottom of this page.

Grant Application Form

Please read the Grant Application Guidelines and Instructions for applying online pages before completing an application form.

Please consider the following questions before proceeding on to the application form; these questions will help you to determine which version of the application is most appropriate for your organization and for your request. 

Is your request for funds from Z. Smith Reynolds Foundation for General Operations (unrestricted funds used for your organization’s overall budget) or for Project Support (funds earmarked for a particular activity or project within your organization)?

To apply for a general operating or project grant, the applicant must complete the online application form appropriate to the type of grant request and the organization. There are two applications from which to choose:

  1. General Operating Support Grant Request
  2. Project Support Grant Request

The applications are tailored for each situation and an applicant will be required to select the appropriate application before beginning the online submission process.

Grantee's Reporting Form
If you are a current grantee, you must complete this form no later than 18 months from the date the grant is paid.

Interim Reporting Form
If you are a current grantee with a multi-year grant, you must submit this Form annually to the Foundation.

Progress Reporting Form
If you are a current grantee and plan to submit another application prior to the completion of your current grant, you must submit this Form to the Foundation along with your new grant application.

Nancy Susan Reynolds Award Nomination Form
Up to three Awards are given annually to “unsung heroes” of North Carolina in the areas of Advocacy, Personal Service and Race Relations.

Sabbatical Application Form

Five NC nonprofit leaders are selected each year to receive a three to six month sabbatical. Please read the "Sabbatical" section of our web page before beginning an application.

    Fellowship Application Form
    The Foundation annually selects a person early in his/her career to serve up to two years on staff.
