Grantee Reporting Forms

1) All grantees are required to complete a Grantee's Reporting Form, which is due no later than 18 months after a grant is paid. If you are a grantee, you may retrieve this form using one of the following options:

Grantee's Reporting Form

    a) online submission via IGAM (coming soon)
    b) download the form on the    Forms and Publications page
    c) request a mailed copy on the   Forms and Publications page

2) If your organization received a multi-year grant, the Foundation requires that an Interim Reporting Form be submitted annually. If you are a grantee, you may retrieve this form using one of the following options:


Interim Reporting Form 

     a) online submission via IGAM (coming soon)
     b) download the form on the     Forms and Publications page
     c) request a mailed copy on the     Forms and Publications page


3) If you are a current grantee and would like to apply for additional funds, you must complete a Progress Report on the prior grant and submit it with our current grant application. You may retrieve this form using one of the following options:

     a) online submission via IGAM (coming soon)
     b) download the form on the     Forms and Publications page
     c) request a mailed copy on the     Forms and Publications page
