State-Level Systemic Change Grantee Acceptance and Understanding (GAU) Form - The grantee shall receive the Grant Award Notification email approximately three to four weeks after the ZSR Board of Trustees meet. At or about the same time, the award notification is emailed and the Grantee Acceptance and Understanding Form (the contract between the Foundation and grantee) will be placed in the grantee’s online account. Reference the Grantee Acceptance and Understanding Form for earliest availabilty of funds. The Grantee Acceptance and Understanding Form is submitted only once for the duration of the grant. Sample State-Level Systemic Change Grantee Acceptance and Understanding (GAU) Form
Diversity Plan Requirement Form – The Grant Resolution indicates if a grantee was required to submit a Diversity Plan. If the grant resolution requires a Diversity Plan to be submitted, the Diversity Plan Form must be submitted online in addition to the GAU Form. The Diversity Plan Form must be received and approved by Foundation staff before the Foundation will release payment of grant.
Matching Grant Requirement Form – If the grant resolution requires a match(s), the grantee has up to 18 months to obtain the matching funds and submit the form.
Interim Reporting Requirement Form - As indicated in the Grantee Acceptance and Understanding (GAU) Form, organizations receiving a State-Level Systemic Change grant for a grant period longer than one year (also known as multi-year grantees) shall submit an Interim Report in the 11th month of each year of the grant period, except in the final year of the grant. The grant period begins in January of the first year of the grant unless otherwise indicated in the Grant Resolution found above these instructions. The Foundation will notify grantees by email when the Interim Report is available in the grantee’s ZSR online account.
The Interim Report must be received and approved by Foundation staff before the Foundation will release subsequent payments.
During the final year of a multi-year grant, the grantee must submit a Final Report within three months from the end of the grant period. The Foundation will notify the grantee by email when the Final Report form is available in the grantee’s online ZSR account.

Final Reporting Requirement Form - The grantee will submit a full and complete written Final Report to the Foundation no later than three months after the end of the grant period. If the grant period is extended, the Final Report will be due three months after the end of the extended grant period. The Final Report will describe the progress made in accomplishing the purposes of the grant and provide a complete financial accounting of grant expenditures in accordance with the agreement.
