For Z. Smith Reynolds Foundation Grantees

In an effort to streamline our requirements and reporting process, the Z. Smith Reynolds Foundation has transitioned from requiring grantees to download paper documents from our website to completing and submitting forms through an online system.

Please do not mail a copy of the signed Grantee Acceptance and Understanding (GAU) Form or email the form to a Program Officer. We will only accept the GAU Form through our online system.

You can now access your saved applications as well as reporting and requirement forms from our grant request portal.

ZSR Online Reporting Requirements Guide For Grantees

As a guide for the grantee, below is a list of the reporting requirements that we require. The required form(s) will be placed in the grantee’s online account. (NOTE:  Review the “Grant Resolution” section of your Grantee Acceptance and Understanding Form to determine if any stipulations have been placed on your grant.)   

ZSR WILL ONLY ACCEPT THE REQUIREMENT FORMS THROUGH OUR ONLINE SYSTEM.

DO NOT MAIL OR EMAIL THESE FORMS TO THE FOUNDATION.

Additional Requested Information Form – After the grant application has been submitted and reviewed by ZSR staff, any missing and/or incomplete information that was not submitted with the application will be submitted by the applicant through this form. 
 
 
Grant Extension Form – Under certain circumstances, a grantee may need more time to complete the grant.  The grantee should contact the Foundation. The Grant Extension Form will then be placed in the grantee’s online account. Once the grantee has submitted, the Foundation will review and notify the grantee if the extension was granted.
